Personalised support in the comfort of home

Living with dementia can make everyday routines and activities more difficult. We provide practical, person-centred support designed to help people remain engaged, comfortable and supported in their own home.

Support may include:

  • Assistance with establishing and maintaining meaningful daily routines

  • Support with meaningful activities and social engagement

  • Strategies to promote independence and participation in everyday activities

  • Support with personal care routines and wellbeing

  • Assistance with meal preparation and maintaining healthy routines

  • Identifying practical changes that can make the home environment safer and easier to manage

  • Support for families to better understand and respond to changing needs

Our focus is on maintaining dignity, independence and quality of life while supporting the person to continue doing the things that matter to them.

Understanding what is behind changes in behaviour

Changes in behaviour can be one of the most difficult aspects of dementia for families and care-partners to understand and manage.

We work with the person and their family to understand what may be contributing to changes in behaviour, distress or communication and develop practical strategies that can be incorporated into everyday life.

Support may include:

  • Understanding changes in behaviour and communication

  • Identifying possible triggers and unmet needs

  • Developing practical strategies to reduce distress

  • Supporting families to respond confidently to challenging situations

  • Adapting routines and environments to better support the person

  • Promoting meaningful engagement and positive interactions

  • Reviewing strategies as needs and circumstances change

The aim is not simply to manage behaviour, but to understand what the person may be communicating through their behaviour and identify approaches that support comfort, dignity and wellbeing.

Helping families feel informed, confident and supported

Dementia affects the whole family. As needs change, families and care-partners can benefit from practical information, guidance and reassurance.

We work alongside families to build their understanding of dementia and develop strategies that make everyday life more manageable.

Support may include:

  • Education about dementia and its impact on everyday life

  • Guidance around communication and responding to changing needs

  • Practical coaching for care-partners and family members

  • Strategies for managing changes in routines and behaviour

  • Support with maintaining meaningful activities and relationships

  • Regular follow-up and review

  • Adjusting strategies as the person's needs change

  • Recommendations and guidance to help families access appropriate supports and services

Our goal is to help families feel less alone, more confident and better equipped to support their loved one.
